Please bring with you or ask your dentist to forward any relevant x-rays. 

For periodontal consultation, an OPG from the last 18 months is sufficient and required. For implant consultation, a CBCT is necessary. 

If not available, relevant x-rays can be taken at our practice for a small fee.
